What the evidence actually covers

The weight-loss results people cite for semaglutide come from trials of the approved product at labelled doses — the STEP programme, and for cardiovascular outcomes, SELECT. Those trials studied a specific, FDA-reviewed formulation. They are the basis for the approved labelling.

They are not evidence about a compounded preparation. A compounded product may contain the same active molecule, but it has not been demonstrated equivalent, its manufacturing has not been reviewed pre-market, and in the case of oral or sublingual compounded forms, its absorption is not established. Any claim that transfers trial results to a compounded product is going further than the evidence does.

The salt-form problem

Compounded semaglutide has been sold in salt forms — semaglutide sodium and semaglutide acetate — that the FDA has specifically warned are different substances from the base molecule in the approved products. This is the single most concrete quality question in compounded semaglutide, and it is checkable: ask for the certificate of analysis and read which form it names.
